TMJ

TMJ (temporomandibular joint) disorders are a family of problems related to your jaw joint. If you have had symptoms like pain or a “clicking” sound, you’ll be glad to know that these problems are more easily diagnosed and treated than they were in the past. These symptoms occur when the joints of the jaw and the chewing muscles do not work together correctly. TMJ stands for temporomandibular joint, which is the name for each joint (right and left) that connects your jaw to your skull. Since some types of TMJ problems can lead to more serious conditions, early detection and treatment are important.

No one treatment can resolve TMJ disorders completely and treatment takes time to become effective. Dr. Urbanek can help you have a healthier, more comfortable jaw.

Trouble with your jaw?

TMJ disorders develop for many reasons. You might clench or grind your teeth, tightening your jaw muscles and stressing your TM joint. You may have a damaged jaw joint due to injury or disease. Injuries and arthritis can damage the joint directly or stretch or tear the muscle ligaments. As a result, the disk, which is made of cartilage and functions as the “cushion” of the jaw joint, can slip out of position. Whatever the cause, the results may include a misaligned bite, pain, clicking, or grating noise when you open your mouth, or trouble opening your mouth wide.

Do You Have a TMJ Disorder?

  • Are you aware of grinding or clenching your teeth?
  • Do you wake up with sore, stiff muscles around your jaws?
  • Do you have frequent headaches or neck aches?
  • Does the pain get worse when you clench your teeth?
  • Does stress make your clenching and pain worse?
  • Does your jaw click, pop, grate, catch, or lock when you open your mouth?
  • Is it difficult or painful to open your mouth, eat, or yawn?
  • Have you ever injured your neck, head, or jaws?
  • Have you had problems (such as arthritis) with other joints?
  • Do you have teeth that no longer touch when you bite?
  • Do your teeth meet differently from time to time?
  • Is it hard to use your front teeth to bite or tear food?
  • Are your teeth sensitive, loose, broken or worn?

The more times you answered “yes”, the more likely it is that you have a TMJ disorder. Understanding TMJ disorders will also help you understand how they are treated.

TMJ Treatment

There are various treatment options for TMJ that Dr. Urbanek can utilize to improve the harmony and function of your jaw. Once an evaluation confirms a diagnosis of TMJ disorder, Dr. Urbanek will determine the proper course of treatment. It is important to note that treatment always works best with a team approach of self-care combined with professional care.

The initial goals are to relieve the muscle spasms and joint pain. This is usually accomplished with a pain reliever, anti-inflammatory, or muscle relaxant. Steroids can be injected directly into the joints to reduce pain and inflammation. Self-care treatments can often be effective as well and include:

  • Resting your jaw
  • Keeping your teeth apart when you are not swallowing or eating
  • Eating soft foods
  • Applying ice and heat
  • Exercising your jaw
  • Practicing good posture

Stress management techniques such as biofeedback or physical therapy may also be recommended, as well as a temporary, clear plastic appliance known as a splint. A splint (or nightguard) fits over your top or bottom teeth and helps keep your teeth apart, thereby relaxing the muscles and reducing pain. There are different types of appliances used for different purposes. A nightguard helps you stop clenching or grinding your teeth and reduces muscle tension at night. It also helps to protect the cartilage and joint surfaces. An anterior positioning appliance moves your jaw forward, relieves pressure on parts of your jaw, and aids in disk repositioning. It may be worn 24 hours/day to help your jaw heal. An orthotic stabilization appliance is worn 24 hours/day, or just at night, to move your jaw into proper position. Appliances also help protect tooth wear.

What about bite correction or surgery?

If your TMJ disorder has caused problems with how your teeth fit together you may need treatment such as bite adjustment (equilibration), orthodontics with or without jaw reconstruction, or restorative dental work. Surgical options, such as arthroscopy and open joint repair restructuring, are sometimes needed, but are reserved for severe cases. Dr. Urbanek does not consider TMJ surgery unless the jaw can’t open, is dislocated and non-reducible, has severe degeneration, or the patient has undergone appliance treatment unsuccessfully.

I initially believed that I’d have my jaw pain the rest of my life and just endure. Then, my dentist mentioned Dr. Urbanek to me but I still didn’t contact his office. When I my primary care doctor referenced him, I followed through with an appointment, still skeptical he could do anything for my symptoms. I’m was so extremely glad and relieved when he gave me an explanation of my symptoms, not only jaw pain, but my headaches, shoulder, neck pains, tinnitus, etc. It has been 6 weeks, and the only symptom remaining is my tinnitus. Although, I still have my white noise, it has decreased in volume. I’m very blessed to have followed through with making my appointment and being under Dr. Urbanek’s excellent wisdom and care! I no longer will accept pain as a normal way of life as I age. Thank you to Dr. Urbanek, and all his caring and polite team!

They ran tests etc, the migraines kept coming I would have anywhere from 4 to 9 or more a month. I am on several meds and was even getting Botox injections every 3 months and was still averaging around 9 migraines a month. Then I came to Dr. Urbanek and everything changed he told me why I was having migraines and other issues and what would stop them. I jus had to give it a try, what did I have to lose? I got fitted for the Splint and once it arrived within a few days I started to notice improvement in my symptoms it was such a major Blessing!!!! My jaw pain was so bad I could barely fit a fork in my mouth. My jaw popped and locked almost every time I ate. I had ringing in my ears, headaches, and neck pain. I was always told all of my symptoms were from past trauma to my head and neck. I came to Dr. Urbanek when I couldn’t stand it any more. Within a week of wearing my Splint, I could tell a big difference. I was able to open my mouth wider and chew without any pain. After 5 months, I am symptom free!!!!

When I came here I could barely open my mouth to sing or eat without feeling locking or pain. That pain had been going on for a month and before that, my jaw had popped my whole life. After just a few days of wearing the Splint, I could open my mouth fully without any pain or pop. This thing worked like magic and even is still getting rid of other symptoms I was unaware were from TMJ, like headaches and a fluid ear feeling. I’m so thankful for it and would recommend to anyone experiencing TMJ symptoms!

As I suffered from my jaw and joint pain, I discovered a friend at church had the same pain. She shared with me the amazing results she had experienced from visiting Dr. Urbanek. I’m so thankful my friend shared her information with me. I thought I would have to live with this chronic pain forever! I have never had issues with my TMJ in past. Well at least I thought that I hadn’t until I found Dr. Urbanek via internet. I started have dizziness issues in October 2017 so I visited my local ENT doctor. I was prescribed medication for my sinus but continued to have dizziness. Shortly the dizziness turned into vertigo and I would be spinning dizzy anytime I would lay down. I then began having major headaches in my joint area along with neck stiffness and jaw pain. I went to my regular doctor who suggested I may have TMD. Searching for a specialist who could resolve my issues I found Dr Urbanek and boy am I grateful I did! He did X-rays on my jaws and quickly discovered I grind my teeth while sleeping. After we discussed my options, I decided to invest in his device, because at this point I was desperate to stop the dizziness. I have to admit that I was a little skeptical at first but I’m glad I gave it a chance. I’ve been wearing the device for two months now and my symptoms are about 85% resolved. So I only have to wear it at night now. I am confident that as long as I keep following Dr Urbanek’s protocol that I will 100%. I have already recommended several people to Dr Urbanek and will continue to recommend because many people are unaware that the symptoms they are experiencing could be related to issues with their TMJ. He is a very thorough, professional, and experienced TMJ Specialist. He is also a very nice and caring person.

I was on the verge of becoming addicted to painkillers before finding Dr. Urbanek. I had a couple of startling events that served to get my attention. I was on my way to work one morning and did not remember getting ready or leaving home. I was taking so much pain medicine that I had no business driving. Since they were prescriptions, it never occurred to me that I could be that “high”. The other event that concerned me was realizing that I would come home after a bad day and take a pain pill. To be fair, the pain was pretty constant but it was the reasoning I used when medicating. This had to stop! I needed help! I began tracking my headaches - when it hurt, where it hurt, etc. I did research on my own. I needed answers - not pills. I found Dr. Urbanek’s website on a Thursday night. I was so excited that I may have found an answer that I couldn’t sleep! I called but they were not open on Fridays. I was so excited the whole weekend! When I finally came in and Dr. Urbanek told me he could help me I cried! I had hope! Now, I’m pain free and grateful every day.
