TMJ

TMJ (temporomandibular joint) disorders are a family of problems related to your jaw joint. If you have had symptoms like pain or a “clicking” sound, you’ll be glad to know that these problems are more easily diagnosed and treated than they were in the past. These symptoms occur when the joints of the jaw and the chewing muscles do not work together correctly. TMJ stands for temporomandibular joint, which is the name for each joint (right and left) that connects your jaw to your skull. Since some types of TMJ problems can lead to more serious conditions, early detection and treatment are important.

No one treatment can resolve TMJ disorders completely and treatment takes time to become effective. Dr. Urbanek can help you have a healthier, more comfortable jaw.

Trouble with your jaw?

TMJ disorders develop for many reasons. You might clench or grind your teeth, tightening your jaw muscles and stressing your TM joint. You may have a damaged jaw joint due to injury or disease. Injuries and arthritis can damage the joint directly or stretch or tear the muscle ligaments. As a result, the disk, which is made of cartilage and functions as the “cushion” of the jaw joint, can slip out of position. Whatever the cause, the results may include a misaligned bite, pain, clicking, or grating noise when you open your mouth, or trouble opening your mouth wide.

Do You Have a TMJ Disorder?

  • Are you aware of grinding or clenching your teeth?
  • Do you wake up with sore, stiff muscles around your jaws?
  • Do you have frequent headaches or neck aches?
  • Does the pain get worse when you clench your teeth?
  • Does stress make your clenching and pain worse?
  • Does your jaw click, pop, grate, catch, or lock when you open your mouth?
  • Is it difficult or painful to open your mouth, eat, or yawn?
  • Have you ever injured your neck, head, or jaws?
  • Have you had problems (such as arthritis) with other joints?
  • Do you have teeth that no longer touch when you bite?
  • Do your teeth meet differently from time to time?
  • Is it hard to use your front teeth to bite or tear food?
  • Are your teeth sensitive, loose, broken or worn?

The more times you answered “yes”, the more likely it is that you have a TMJ disorder. Understanding TMJ disorders will also help you understand how they are treated.

TMJ Treatment

There are various treatment options for TMJ that Dr. Urbanek can utilize to improve the harmony and function of your jaw. Once an evaluation confirms a diagnosis of TMJ disorder, Dr. Urbanek will determine the proper course of treatment. It is important to note that treatment always works best with a team approach of self-care combined with professional care.

The initial goals are to relieve the muscle spasms and joint pain. This is usually accomplished with a pain reliever, anti-inflammatory, or muscle relaxant. Steroids can be injected directly into the joints to reduce pain and inflammation. Self-care treatments can often be effective as well and include:

  • Resting your jaw
  • Keeping your teeth apart when you are not swallowing or eating
  • Eating soft foods
  • Applying ice and heat
  • Exercising your jaw
  • Practicing good posture

Stress management techniques such as biofeedback or physical therapy may also be recommended, as well as a temporary, clear plastic appliance known as a splint. A splint (or nightguard) fits over your top or bottom teeth and helps keep your teeth apart, thereby relaxing the muscles and reducing pain. There are different types of appliances used for different purposes. A nightguard helps you stop clenching or grinding your teeth and reduces muscle tension at night. It also helps to protect the cartilage and joint surfaces. An anterior positioning appliance moves your jaw forward, relieves pressure on parts of your jaw, and aids in disk repositioning. It may be worn 24 hours/day to help your jaw heal. An orthotic stabilization appliance is worn 24 hours/day, or just at night, to move your jaw into proper position. Appliances also help protect tooth wear.

What about bite correction or surgery?

If your TMJ disorder has caused problems with how your teeth fit together you may need treatment such as bite adjustment (equilibration), orthodontics with or without jaw reconstruction, or restorative dental work. Surgical options, such as arthroscopy and open joint repair restructuring, are sometimes needed, but are reserved for severe cases. Dr. Urbanek does not consider TMJ surgery unless the jaw can’t open, is dislocated and non-reducible, has severe degeneration, or the patient has undergone appliance treatment unsuccessfully.

Thirty years ago, I bit into a thick sandwich with my mouth wide open, that resulted on an abnormal sound in the back of my jaw. From them began a discomfort that gradually increased with time. The dental professional I saw couldn’t help me until my new dentist in franklin, tan recommended Dr. Tony Urbanek. My symptoms included consistent headaches( almost daily), pain when biting and eventually my teeth and jaws no longer aligned with each other. Dr Urbanek explained that after so many years of not finding care, my jaws were permanently damaged, after taking x-rays. The splint he developed for me eventually got rid of my symptoms. Having occasional headaches, was huge for me. Not to mention my bite corrected. Please don’t wait to see Dr. Urbanek! And thank you Dr. Urbanek.

Several years in a typical night guard to protect against teeth grinding just didn’t do it fo me. After consulting with my dentist we decided that Dr Urbanek just might be a good fit for me. I came in with many of the typical symptoms and after several months treatment with Dr Urbanek’s device, I feel so much better and can even see the difference in my jawline. So glad I tried this treatment.

I’ve been grinding my teeth since I was a child, and over time it has lead to jaw problems that became very painful. The Splint has really decreased much of my pain. While my jaw still catches and locks sometimes, it is not nearly as much as it used to and it doesn’t hurt! I know my jaw will never be “normal” but this device really seems to have made this problem much more manageable, and I like knowing that the device stops my back teeth from grinding at night. I look forward to seeing my issues decrease even more over the coming months!

I got my splint made 5 years ago from Dr. Urbanek for the reason of me grinding my teeth together during the night. I tried several different brands of mouth guards and splints but none of them compared to this one. It has made a big difference for me and I’d hate to be without it.

I have suffered TMJ pain for almost 30 years. As an adult I have had expanders, numerous sets of braces & retainers. The pain would still come and go regardless of the treatment(s). I began grinding my teeth in my sleep about 10 years ago & broke 2 molars. I then began grinding my teeth all hours of the day & had horrible jaw pain, locking & spasms. The pain was horrible & I knew that I was over using Tylenol & Aleve. After wearing Dr. Urbanek’s TMJ Splint for just 1 day the pain was gone! I noticed that I no longer have neck & shoulder pain or headacheseither. My only regret is not finding Dr. Urbanek sooner!!

My experience with Dr Urbanek was wonderful. He explained to me that this was a process and it would take time, but for me it took me no time at all. I had to go the first three weeks taking ibuprofen and Tylenol and keeping something warm on my jaw and I could only eat potato soup(chili’s has the best). But now I only have 10% of the pain and problems that I started with.

After two months of wearing my new appliance, my symptoms of headache, jaw pain and neck pain are gone. I had been experiencing these symptoms for almost 20 years. I am also sleeping better now. The adjustment of wearing the appliance was certainly easier than expected and well worth the short time it took to be free of discomfort.

I was sent to Dr. Urbanek way back in 1991 because of TMJ problems. The Temporandibular disk on both jaws had to be removed. I remember Dr. Urbanek saying, "If you do everything I tell you to do, you'll be able to eat anything you want". Yes Dr. Urbanek knew what he was talking about. How many years now? 28 years later, I'm eating anything I want. Thank you Dr. Urbanek. I'll always be Thankful to you for taking such good care of me.

For years I struggled with my jaw pain; laughing, yawning, singing, and even just talking for long periods of time would bring on a list of painful symptoms. 12 weeks ago I couldn’t sleep at night due to intense TMJ disorder. My neck and back hurt so bad I couldn’t turn my head from left to right...and that’s the flare up that made me realize I had to do something. Someone referred me to Dr. Urbanek and I’ve been progressively better since the first day I wore his splint. Thank you, Dr. Urbanek for giving me my life back.

I have dealt with TMJ symptoms for forty plus years. I was referred to Dr. Urbanek after talking with my ENT Doctor. He told me about the success that Dr. Urbanek was having with a splint that he had developed. I started using the splint in Sept 2018. The jaw pain, headaches, dizziness, and ringing in my ears has diminished to the point I now have pain/dizzy free days. Thank you to Dr. Urbanek and staff.
