TMJ

TMJ (temporomandibular joint) disorders are a family of problems related to your jaw joint. If you have had symptoms like pain or a “clicking” sound, you’ll be glad to know that these problems are more easily diagnosed and treated than they were in the past. These symptoms occur when the joints of the jaw and the chewing muscles do not work together correctly. TMJ stands for temporomandibular joint, which is the name for each joint (right and left) that connects your jaw to your skull. Since some types of TMJ problems can lead to more serious conditions, early detection and treatment are important.

No one treatment can resolve TMJ disorders completely and treatment takes time to become effective. Dr. Urbanek can help you have a healthier, more comfortable jaw.

Trouble with your jaw?

TMJ disorders develop for many reasons. You might clench or grind your teeth, tightening your jaw muscles and stressing your TM joint. You may have a damaged jaw joint due to injury or disease. Injuries and arthritis can damage the joint directly or stretch or tear the muscle ligaments. As a result, the disk, which is made of cartilage and functions as the “cushion” of the jaw joint, can slip out of position. Whatever the cause, the results may include a misaligned bite, pain, clicking, or grating noise when you open your mouth, or trouble opening your mouth wide.

Do You Have a TMJ Disorder?

  • Are you aware of grinding or clenching your teeth?
  • Do you wake up with sore, stiff muscles around your jaws?
  • Do you have frequent headaches or neck aches?
  • Does the pain get worse when you clench your teeth?
  • Does stress make your clenching and pain worse?
  • Does your jaw click, pop, grate, catch, or lock when you open your mouth?
  • Is it difficult or painful to open your mouth, eat, or yawn?
  • Have you ever injured your neck, head, or jaws?
  • Have you had problems (such as arthritis) with other joints?
  • Do you have teeth that no longer touch when you bite?
  • Do your teeth meet differently from time to time?
  • Is it hard to use your front teeth to bite or tear food?
  • Are your teeth sensitive, loose, broken or worn?

The more times you answered “yes”, the more likely it is that you have a TMJ disorder. Understanding TMJ disorders will also help you understand how they are treated.

TMJ Treatment

There are various treatment options for TMJ that Dr. Urbanek can utilize to improve the harmony and function of your jaw. Once an evaluation confirms a diagnosis of TMJ disorder, Dr. Urbanek will determine the proper course of treatment. It is important to note that treatment always works best with a team approach of self-care combined with professional care.

The initial goals are to relieve the muscle spasms and joint pain. This is usually accomplished with a pain reliever, anti-inflammatory, or muscle relaxant. Steroids can be injected directly into the joints to reduce pain and inflammation. Self-care treatments can often be effective as well and include:

  • Resting your jaw
  • Keeping your teeth apart when you are not swallowing or eating
  • Eating soft foods
  • Applying ice and heat
  • Exercising your jaw
  • Practicing good posture

Stress management techniques such as biofeedback or physical therapy may also be recommended, as well as a temporary, clear plastic appliance known as a splint. A splint (or nightguard) fits over your top or bottom teeth and helps keep your teeth apart, thereby relaxing the muscles and reducing pain. There are different types of appliances used for different purposes. A nightguard helps you stop clenching or grinding your teeth and reduces muscle tension at night. It also helps to protect the cartilage and joint surfaces. An anterior positioning appliance moves your jaw forward, relieves pressure on parts of your jaw, and aids in disk repositioning. It may be worn 24 hours/day to help your jaw heal. An orthotic stabilization appliance is worn 24 hours/day, or just at night, to move your jaw into proper position. Appliances also help protect tooth wear.

What about bite correction or surgery?

If your TMJ disorder has caused problems with how your teeth fit together you may need treatment such as bite adjustment (equilibration), orthodontics with or without jaw reconstruction, or restorative dental work. Surgical options, such as arthroscopy and open joint repair restructuring, are sometimes needed, but are reserved for severe cases. Dr. Urbanek does not consider TMJ surgery unless the jaw can’t open, is dislocated and non-reducible, has severe degeneration, or the patient has undergone appliance treatment unsuccessfully.

Excellent solution to a long-standing and painful problem. For 45 years I had headaches, jaw pain and other symptoms that I had no idea were associated with TMJ issues. I was referred to Dr Urbanek who after discussing my issues presented me with an solution for my problems. It didn’t take long after I started to use the appliance that I began to experience relief. After wearing the appliance for almost three months about 80% of my symptoms have been relieved.

At first I had serious head aches, jaw pain, tingling and numbness of the mouth /tounge. Also popping of the jaw, neck/shoulder pain, tingling and numbness of the hands, lack of sleep and burning/itching of the eyes. But now I’m only experiencing a little tiredness of the jaws when I remove the mouth piece. I’ve only been using it for 3 months so far 24/7, now I’m going to only at nights. Will see in 2 months if that goes away also. Take it from me try it and you will see the relief you will experience.

My experience was great! Before I came I was having extreme pain in my shoulders and my jaw would lock all the time and most of the time it was very difficult to eat because I either couldn’t open my mouth wide enough or couldn’t chew without being in pain. But after I came here and got my jaw checked out and got my mouthpiece my pain has gone away and I feel better than ever!

Dr. Urbanek was a lifesaver. I had been clinching my teeth for years and eventually, my jaws began to pop out of alignment. He diagnosed me with TMJ. I have faithfully worn my device with a few exceptions and my jaws have not locked up since. I will continue to wear the device at night as prescribed and I look forward to the continued results. This problem didn’t begin overnight so it won’t go away overnight either but I’m much better because of Dr. Urbanek.

I’ve had symptoms going back about 20 years. I finally broke down when I could barely chew solid food, and went to an orthodontist. He fixed my bite, and it helped my symptoms, but they didn’t totally go away. A couple of years post braces, I was back to barely being able to chew. I was referred to Dr. Urbanek, and it changed my life. Within 2 days of wearing the device, the clicking and popping was gone. It’s been about 2 months now and my symptoms have almost totally cleared. No more jaw pain, popping, and headaches. I highly recommend Dr. Urbanek and his team.

After 62 years of marriage, my husband no longer has to hear gunshot noise when I eat . Likewise, I no longer have severe jaw pain and headaches even when not eating. This small device took away the agony almost magically. We moved to Nashville 27 years ago. I know I complained to dentist #1 and dentist #2 and dental consultant #3 and dentist #4. Finally,#4 sent me to Dr.Urbanek. And that solved the at least 70 year problem

I had severe TMJ inflammation when I heard about Dr Urbanek. It was affecting what I ate and how I functioned daily. I had been using a night guard for 6+ years and it was not stopping the downward spiral of issues including ear aches, teeth and jaw pain, headaches, tightness in my neck and shoulders and even tingling in my fingers. On several occasions I was able to do nothing except pack my jaws with ice packs. Im so thankful that a friend shared with me about someone who dealt directly with this issue.

This is the best referral I have ever had in my life. So many symptoms I had, no one could explain. I finally found the piece of the puzzle that was missing. My other Physicians were pleased also, now that we have answers. I have had unexplained tingling in my jaw and shoulder, a feeling of fullness in my head, headaches, tinnitus, my jaw felt misaligned and the list continues. I am grateful for my TMJ splint. Most of my symptoms have been resolved or a great improvement in just 3 weeks. My splint is comfortable and I don't mind wearing it. The results are fabulous. Dr. Urbanek is very competent and nice as well as his office staff. I am one happy patient!
